The Epic Patient Wellness and Care Management Analyst plays a key role in providing the highest quality of patient care while the patient is in at Union Health.
The Epic Patient Wellness and Care Management Analyst supports the implementation, optimization, and maintenance of the Epic Healthy Planet module, focused on patient wellness, care management, and population health initiatives. The analyst is responsible for design, build, and troubleshooting related to the application. This role partners with care managers, clinicians, and IT teams to ensure Healthy Planet meets clinical needs, regulatory requirements, and operational goals.

EPIC Patient Wellness and Care Management Analyst Details
Essential Job Duties:
  • Collaborate with clinical and care management teams to gather requirements and translate them into effective system configurations.
  • Configure and optimize Epic Healthy Planet module to support patient wellness, risk stratification, care management, and outreach programs.
  • Engage in the development and management of patient registries, gaps-in-care alerts, and preventive care initiatives.
  • Ensure compliance with regulatory and institutional policies related to Patient Wellness and Care Management. Including quality reporting initiatives such as PQRS, SCOs/MSSP, and MU.
  • Monitor and report on population health metrics and program effectiveness. Utilize reporting tools to direct optimization efforts and resolve issues.
  • Demonstrate knowledge of Patient Wellness and Care Management workflows to inform system design and optimize clinical practice.

Qualifications:
  • Associate or bachelor's Degree Computer Sciences/Information Systems/Business major or equivalent experience with 5+ years' direct experience working with large scale enterprise information systems is required.
  • Epic Healthy Planet Certification
  • One to two years of Epic application support experience preferred.
  • Functional understanding of analyst's specialty workflows.
  • Progressing towards certification is encouraged.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively with cross-functional clinical and technical teams.
  • Adaptability to changing operational and regulatory needs.
  • Possesses a strong sense of ownership and accountability of work activities to ensure they are completed in an accurate, efficient, and timely manner.
  • Resilient under pressure, with the ability to maintain high performance in challenging or time-sensitive situations.
  • Clinical background is a plus.
