Job Description

The RN Acute Care Service Coordinator partners with the IAC Medical Director to build one world-class integrated acute care team across the care continuum, ensuring safe & controlled care transitions and implementing high-functioning, cost-positive protocols & quality measures. The role comprises three fundamental capabilities: practice management, opportunity identification, and model implementation/change acceleration.

Location: Ascension St. John Medical Center

ESSENTIAL JOB FUNCTIONS:
  • Supports the Medical Director in deploying a well-structured site operating rhythm for the IAC team (Physicians, APPs, Scribes, and Administrative Assistants), including but not limited to:
    • Daily provider census-driven workflow process
    • Rounding / Discharge Huddles and Care Coordination
    • Support across ED, HM and Post-Acute
    • Driving site process around accountable task management (Action Register) and delivery dates
    • Reinforcing clear expectations with the clinical team
  • Works with site administration and clinical team to identify, plan, and drive improvements in a project management environment.
    • Readmission reduction
    • ED time and efficiency metrics
    • Patient experience
  • Under the direction of the USACS Bundled Payments for Care Improvement Advanced leader, collaborates closely with patients and their healthcare teams to ensure efficient, effective, and patient-centered care coordination during care transitions.
  • Quick to identify issues and develop thoughtful opinions; data analysis, as well as the ability to navigate the hospital organization expertly and bring resources to the table
  • Act as an agent of change and become the hospitals' point of contact for a difficult-to-manage situation

EDUCATION AND EXPERIENCE:
  • BSN required.

  • Clinical experience, i.e., Emergency / Outpatient, Inpatient, and Post-Acute settings, a plus.
PHYSICAL DEMANDS:
  • While performing the duties of this job, the employee is regularly required to sit for prolonged periods and occasionally walk, stand, bend, stoop, and lift to 15 pounds.

Salary Range: $78,566-$145,347.10
