Overview
Part time RN (45 hours a pay period)

Team 4: Amityville, Copiague, Farmingdale, Lindenhurst, Wheatley Heights, & Wyandanch.
  • 8:30AM to 4:30PM - Rotating Weekends
  • Great Healthcare benefits, and education reimbursement of $5,250 annually
  • Additional compensation for visits over your caseload

Job Details
Our Home Care RN's:
  • Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients.
  • Measure vital signs, Administer IVs, change dressings, clean wounds as needed
  • Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.
  • Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload.
  • Provide patient and family education for disease management and health promotion as directed by physician's plan of treatment.
  • Provide care to the most upmost standard

Minimum Qualifications:
  • New York State RN license
  • Minimum of one year of recent hands on experience as a Registered Nurse.
  • Basic Life Support Certification
  • Valid New York State Driver's License and reliable transportation

Posted Salary Range
USD $100,000.00 - USD $130,000.00 /Yr.
This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate's qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
