Job description

What You Will Do to Change Lives
At Telecare, we don't just provide services; we help people reclaim their hopes and dreams. As a Case Manager & Substance Use Counselor, you will be the primary advocate for individuals navigating the complexities of mental illness and addiction.
Working at Kearny Vista Permanent Supportive Housing, you'll meet clients where they are-literally and figuratively. Whether you're leading a group session in a community space or providing 1-on-1 support in the field, your presence provides the stability they need to live independently and thrive.
What Your Day-to-Day Looks Like
  • Empowerment: Assist residents in maintaining housing stability through personalized behavioral health screenings and care coordination.
  • Advocacy: Act as the primary case manager, connecting clients to the resources they need to stay healthy and safe.
  • Connection: Facilitate community-building events like BBQs, tenant education, and support groups to foster a sense of belonging.
  • Mobility: Use your personal vehicle to help clients get to vital appointments (with full mileage reimbursement!).

Schedule: Full-Time | Monday to Friday | 10:30 AM - 7:00 PM
Expected starting wage range is $26.50 - $32.50/hr. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.
What You Bring to the Table (Must Have)
  • High School Diploma or G.E.D and Certified Advanced Alcohol and Drug Counselor (CADC), or Certified Addiction Treatment Counselor (CATC), Substance Use Disorder Certified Counselor (SUDCC, SUDCC II, III, IV), or Chemical Dependency Professional (CDP in the state of WA). In AZ must be certified as Minimum state requirements as a Licensed Independent Substance Abuse Counselor (LISAC), Licensed Associate Substance Abuse Counselor (LASAC) or Licensed Substance Abuse Technician (LSAT)
  • Six (6) months' experience in addictions setting
  • All certifications must be maintained for duration of employment
  • Within two (2) months of employment, ability to apply financial requirements for billing and learn billing codes
  • Sensitivity to multi-cultural populations and issues
  • Must be credentialed as a qualified mental health professional, where applicable
  • Must be at least 18 years of age
  • Must be CPR, First Aid, and CPI certified (hands-on course) on date of employment or prior to providing direct client care and maintain current certification throughout employment
  • Valid and current driver's license, and personal vehicle insurance with your name listed as a driver.
  • Willingness to use your personal vehicle to drive clients to appointments and groups, etc. (weekly mileage reimbursement at the IRS rate)
  • Willingness to use your personal vehicle to attend meetings, etc. (weekly mileage reimbursement at the IRS rate)

Service participation is voluntary and not a requirement of residing at Kearny Vista. Telecare works with tenants to engage them in services according to their behavioral health and case management needs. Services are provided throughout the community based on preference: on-site in Kearny Vista PSH community space; in individual units; in the supportive service office; or in the community, if appropriate. These services focus on housing stability and retention, including but not limited to: 1)Outreach and engagement services addressing housing stabilization and case management needs, 2) Behavioral health assessment, screening, care coordination, and referrals for residents at high risk, in need of more intensive behavioral health services to maintain housing, 3) Addressing ongoing behavioral health care coordination needs, and 4) Offering site-wide community integration opportunities (BBQs, celebrations, tenant education, and other groups).
