This is relationship-based care that goes beyond the exam room, supporting patients facing complex medical, behavioral health, and social challenges while working alongside an interdisciplinary team committed to compassionate, patient-centered care.

Our small, collaborative primary care clinic serves individuals living with chronic illnesses, disabilities, behavioral health conditions, and significant social barriers to care. We believe healthcare should meet patients where they are, and we're seeking a Nurse Practitioner who brings empathy, resilience, humility, and a genuine commitment to serving vulnerable communities.

Compensation Range: $103,043.00 to $154,563.00 The anticipated base salary range represents the Company's good-faith estimate of the compensation it reasonably expects to pay for this position at the time of posting. Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.
