Job description

Purpose:

  • Provide case management services for the clients of The Salvation Army Harbor Light

Summary:

  • Facilitate didactic groups and case management services

Primary Duties and Responsibilities:

  • Conducts didactics/educational lectures
  • Maintain case records in accordance with Harbor Light standards
  • Serve as Case Manager to treatment clients
  • Assists clinical staff with various tasks as assigned (i.e. make packets, assist clients with nonclinical issues, etc.)
  • Participates in the review of all clinical issues, activities and program policies and procedures
  • Attends all clinical staff meetings, and team conferences
  • Participates in educational courses, seminars, workshops, current literature and in-service meetings to enhance treatment skills

Qualifications:

  • Ability to determine needs and to work with multi-problem individuals
  • Ability to develop rapport, trust and confidence in client relationships
  • Must possess good communication and teamwork skills
  • Strong organizational skills
  • TB Test and police clearance required
  • Authorization to work within The United States

Education/Experience:

  • A bachelor's degree in social work or an equivalent comparable human services field is required
  • Experience in case management preferred
  • Experience with mental health and/or substance abuse populations preferred

Computer Skills:

  • Excellent computer skills required
  • Experience using electronic medical records systems preferred

Reasoning Ability:

  • Understand and work within The Salvation Army and Harbor Light Mission Statements
  • Possess an excellent working knowledge of The Harbor Light Policies and Procedures

Certificates and Licenses:

  • Must possess MCBAP credential (CADC, CADC-M, CAADC) or have an approved Development Plan within 2 weeks of hire date
  • Valid Michigan Chauffeur License and approved by The Salvation Army MVR
