The Case Manager Assistant provides residents with exceptional case management services that exceed the requirements of all regulatory agencies to include the Texas Department of Family and Protective Services (TDFPS) residential childcare contract, Health and Human Services Commissions (HHSC) minimum standards and Commission on Accreditation of Rehabilitation Facilities (CARF). The services must support the clients service plan and childs plan of service requirements to include: health care, life skills programs, mental wellness, academic programs, and vocational and recreational activities, cultural competence, maintaining connections and contact with siblings and family visits. The Case Manager Assistant works closely with the Case Manager and Quality Assurance and Improvement Coordinator and Licensed Childcare Administrator to ensure that high quality, cost effective programs are designed and implemented with regulatory compliance. Assures the continuity of care for clients, appropriate and timely healthcare referrals and provides necessary information and documentation for admission, discharge and or/transition to receiving units, facilities or agencies.
RESPONSIBLITIES:
  • Provide services including transporting as needed for assigned youth included but not limited to: scheduling and attending appointments to meet the needs of the residents, i.e. education, medical, dental, psychological evaluation, psychiatric and medical hospital admittance, local juvenile authority and family visits, etc.
  • Has comprehension and knowledge of all regulatory requirements to include accreditation, contracts and minimum standards with target goal of 100% compliance.
  • Maintain and audit all case files to ensure they are complete, accurate and updated timely to meet minimum standards, as well as filing documents, setting up new files and "thinning" cases.
  • Coordinate educational services provided by academic school, extra-curricular activities and ensure required documentation is provided timely for enrollment, discipline, etc..
  • Assist in school enrollment of HWHF clients.
  • Implement any restrictions to communication and visitation with the residents family that are imposed on a resident but have not been reviewed and approved by the treatment director or service planning team, including monthly re-evaluations of restrictions that continue for more than 30 days.
  • Familiarize and educate youth on Transitional Support Services
  • Coordinate and process admission and discharge youths
  • Coordinate and support all educational services to include enrollment, ARDS, etc.
  • Ensure audits of medication (psychotropic and over the counter) and first aid kits are being completed and accurate;
  • Familiarize yourself with the STAR Health services to support the youth for cross-system coordination.
  • Adhere to all regulations and standards necessary to meet licensing and accreditation compliance.
  • Coordinate and process admission and discharges of clients.
  • Assure the continuity of care for clients, appropriate health referrals and provides necessary information and documentation.
  • Backup to Case Manager in sending medication orders to pharmacy, perform accurate inventories, prepare destruction logs and prepare medication sheets timely and efficiently to meet 100% compliance at all times.
  • Safeguard privacy, confidentiality and timely accuracy of all client records and conversations.
  • Complete a minimum of 50 hours on in-service training hours annually in child-care services, excluding first aid and orientation.
  • Coordinate Psychiatric and Medical Hospitalizations (all steps of the process).
  • Communicate effectively with others and deter gossip and negativity that impacts clients, staff and the workplace;
  • Meet deadlines as required per our multiple contracts (DFPS, OCOK, Family Tapestry, Saint Francis and Post Adoption), CARF, Minimum Standards and YFT.
  • Other duties as assigned.
