New Fordham Arms, a 35-bed Adult Care Facility in the Bronx, is looking for a sharp, organized Case Manager with financial case management experience to join our team. This is a dual-function role — you'll be coordinating resident care and services while also managing resident personal allowance accounts and benefits administration. If you're someone who thrives in a hands-on environment and genuinely cares about the people you serve, we'd love to hear from you.

Job Responsibilities:

  • Conduct intake assessments and develop individualized service plans for residents

  • Coordinate and supervise clinical care, including admissions and re-admissions from the community, hospitals, and nursing homes

  • Communicate with residents' care providers and families to ensure continuity of care

  • Manage resident personal allowance (PNA) accounts in compliance with NYS DOH regulations

  • Assist residents with benefits applications including SSI, SSP, and OTDA; coordinate with the Social Security Administration as needed

  • Maintain accurate documentation in ALIS in accordance with facility and regulatory requirements

  • Uphold each resident's Bill of Rights at all times

Qualifications:

  • Minimum 1 year of experience as a Case Manager required; experience in an ACF, LTC, or adult care setting strongly preferred

  • Familiarity with resident financial administration, benefits coordination, or Social Security Representative Payee procedures a plus

  • Experience with ALIS or similar resident management software preferred

  • Strong organizational skills with the ability to manage both clinical and administrative responsibilities
