Responsibilities:

  • Assess potential Hospice or Home Health patients in a timely manner
  • Educate patient, family, and caregivers on Hospice and Home Health services
  • Conduct patient assessments to determine their needs and develop individualized care plans
  • Include the patient and caregiver in the development of the care plan
  • Collaborate with healthcare professionals to coordinate diagnostic evaluations and treatment plans
  • Provide direction to LPN to ensure patients needs are met
  • Utilize knowledge of physiology and medical conditions to provide appropriate care and interventions
  • Perform educational visits to potential patients, providers, or facilities as requested
  • Participate in IDT and assist in Home Health to Hospice conversions
  • Conduct utilization management activities to ensure appropriate use of healthcare resources
  • Provide education and support to patients and their families regarding their healthcare options and treatment plans
  • Coordinate delivery of needing supplies, equipments, and medications in a timely manner
  • Provide education to community partners on hospice and home health and assist in business development
  • Perform Case Management responsibilities including but not limited to updating plan of cares, performing recertifications, and supervisory visits.

Skills:

  • In-depth knowledge of physiology and medical conditions, allowing for accurate assessment and intervention for both Hospice and Home Health
  • Proficient in conducting patient assessments and utilizing diagnostic evaluations to inform care plans
  • Excellent patient service skills, with the ability to provide compassionate and comprehensive care
  • Experience in hospice and good understanding of qualifications for hospice.
