What is a case manager?

Case managers are professionals who coordinate and manage support services for individuals in need, such as patients, clients, or social service recipients. They assess clients’ needs, develop care plans, and connect them with appropriate resources to improve their well-being. Case managers often work in healthcare, social services, or mental health settings and act as advocates to ensure clients receive comprehensive and effective support. Their goal is to help clients achieve the best possible outcomes through continuous monitoring and adjustment of care plans.

What do case managers do?

A case manager is a patient care professional who assesses and oversees a patient’s or client’s complete case. Case managers coordinate the many providers involved in a patient’s or client’s care. Depending on the particular position, this may mean coordinating social services, rehabilitation and therapy services, home healthcare, in-patient care, and more. Above all, case managers see that the needs of their patients' or clients' are understood clearly and met as best they can be.

What are the key skills and qualifications needed to thrive as a case manager, and why are they important?

To thrive as a Case Manager, you need strong organizational skills, a background in social work or a related field, and typically a bachelor's degree or relevant certification such as CCM (Certified Case Manager). Familiarity with case management software, electronic health records, and documentation systems is essential for managing client information efficiently. Outstanding communication, problem-solving, and empathy are vital soft skills for building trust and advocating for clients' needs. These competencies are crucial to coordinating resources, ensuring client well-being, and achieving successful outcomes in complex cases.

How does a case manager typically collaborate with other professionals to support clients?

Case Managers frequently work as part of a multidisciplinary team that may include social workers, healthcare providers, mental health professionals, and community resource coordinators. Regular communication and coordination are essential, as Case Managers often organize case conferences, share client progress updates, and advocate for client needs across various services. Collaborating effectively ensures that clients receive comprehensive and cohesive support, making teamwork and strong interpersonal skills critical for success in this role.

Job description

Description:

About SilverStay

SilverStay is on a mission to revolutionize hospital discharge. We help hospitals, families, and post-acute providers work together to ensure even the most complex patient discharges happen smoothly, quickly, and compassionately.  Our model blends embedded hospital support with advanced data tools to reduce length of stay (LOS), alleviate weekend discharge bottlenecks, and transform how care transitions happen. We are seeking Case Managers (LCSWs, MSWs, & RNs) to manage complex discharges. This role is ideal for a motivated, outcomes-driven professional who thrives in fast-paced environments and wants the flexibility, creativity, and autonomy to improve how patient discharges are handled. Unlike traditional hospital case management, this role gives you the chance to innovate, collaborate, and directly impact patients and families during critical transitions of care.

What You’ll Do

  • Facilitate safe and successful discharges for complex patients by navigating barriers and driving innovative solutions across hospital teams and post-acute care settings within SilverStay’s preferred provider network — including SNFs, LTACHs, rehab, home health, and community resources.
  • Use creativity and resourcefulness to solve discharge challenges without the constraints of hospital bureaucracy.
  • Participate in team rounds, care team huddles, and engage with patients and families to ensure seamless communication across all stakeholders.
  • Present reports to hospital leadership on progress, scalability efforts, and results, proposing adjustments as needed.
  • Build and refine scalable workflows and processes that increase efficiency and outcomes. Partner with SilverStay leadership and hospital teams to track key metrics (LOS, readmission rates, discharge success) and continuously improve results.
  • Leverage technology and data-driven insights to transform how complex cases are managed.

Why Join SilverStay

  • Autonomy & flexibility: This isn’t a rigid 7-to-7 hospital shift. You’ll have the freedom to manage your time and approach, as long as you deliver results.
  • High expectations + high support: We set ambitious goals, but we also make sure our team has the tools and backup to achieve them.
  • Growth potential: No cap to your impact, no cap to your salary. As SilverStay grows, so can you.
  • Values-driven: Empathy. Creativity. Collaboration. Ethical Conduct. And always, wowing patients and clients.
  • We take care of our people: When you need help, you’ll get an answer right away. We prioritize responsiveness, support, and being genuinely good colleagues.

Compensation & Benefits

  • Competitive Salary: $90,000 - $130,000; commensurate with experience and potential impact. ? Performance-Bonus: Bonus structure that can add 10% to 40% of base salary, tied directly to outcomes and results. Paid out quarterly.
  • Health & Wellness: Comprehensive medical, dental, and vision coverage - including spouse and family.
  • Retirement Planning: 401(k) with employer contribution.
  • Paid Time Off: Generous PTO plus holidays.
  • Flexible schedule with autonomy to structure your time around outcomes, not shifts. ? Professional Development: Ongoing training, mentorship, and opportunities to grow into leadership as SilverStay expands.

  • Experience using EMR referral platforms such as Telligen, Careport/WellSky, EPIC, and Cerner.
  • A patient and family-centered communicator with empathy, professionalism, and persistence.
  • Ambitious and entrepreneurial — eager to grow, innovate, and take ownership of your work.
