Job Description:
The RN (Registered Nurse), Case Manager is responsible for managing the overall patient care process, focusing on acute inpatient case management and discharge planning. The role requires strong experience in working in a large, busy hospital environment.
Responsibilities:
  • Manage a case load of 15-20 patients per day, ensuring efficient discharge planning for a wide range of medical diagnoses.
  • Consult with various departments including social work, respiratory therapy, and pharmacy as needed for discharge planning.
  • Work collaboratively with a team of full-time and part-time social workers to deliver comprehensive care.
Requirements:
  • Minimum 2 years of acute inpatient medical case management experience, with at least 1 year in the last 3 years in a large, busy hospital ED (500+ beds, Level 1/Academic Medical Center).
  • Recent acute care discharge planning experience within the last two years.
  • Strong computer skills and proficiency in EPIC charting.
  • BLS Certification is required.
