Job description

RN Case Management/Discharge Planning

This position involves case management and discharge planning for a variety of patients. Candidates will be required to manage an average caseload of 15-20 patients per day. RNs will collaborate with a social work team consisting of multiple full-time and part-time professionals. The role includes covering all inpatient units across the facility. Candidates must be able to pick up quickly and work efficiently, as training on the unit will be minimal. Collaboration with respiratory therapy and pharmacy will be necessary as needed for discharge planning. Candidates can expect to regularly consult with other team members to ensure comprehensive patient care. Emergency department experience and familiarity with complex patient placements are critical for this role. Candidates will work under a schedule of Monday through Friday with hours from 8 AM to 4:30 PM. No weekend or holiday requirements are included in this role.

Minimum 2 years of Acute Inpatient Medical Case Management experience, including at least 1 year in a large busy hospital with a minimum of 500 beds. Active RN license required. BLS Certification required.

Must-Have:

  • Recent acute care discharge planning experience for patients with a wide range of medical diagnoses
  • Strong experience in acute inpatient case management and discharge planning
  • Experience in a level 1 facility within the last 3 years
  • Strong computer skills with proficiency in EPIC charting required
