The Manager, Rehab Case Management is responsible for the administrative management including the clinical, operating and financial outcomes for the case management department. Works in collaboration with customers (patients, families, volunteers, etc.), administration, medical staff, peers, and staff to achieve departmental goals and objectives. In collaboration with the Rehab leadership team, this position shares responsibility for strategic planning, program development, patient and family experience, standards compliance and integration of system wide service delivery.
