RN Case Manager Opportunity – Lodi, California

Assignment Dates: April 6, 2026 – July 5, 2026
Location: Lodi


Position Summary

Step into a rewarding RN Case Manager role where your expertise directly impacts patient outcomes and care continuity. You’ll be at the center of care coordination partnering with physicians, families, and interdisciplinary teams to ensure patients receive the right care at the right time.


Schedule & Assignment Details
  • Shift: Day Shift (8:00 AM – 6:30 PM)

  • Hours: 40 hours/week

  • Shift Length: 10 hours

  • Days/Week: Variable (includes weekends)

  • Duration: 13 weeks

  • Lead coordination of patient care across interdisciplinary teams

Requirements

Education & Experience:

  • BSN (preferred)

  • Minimum 1 year acute care case management experience

Licensure & Certifications:

  • Active RN license (required)
