We are currently seeking a Full-Time Home Health Registered Nurse (RN) Case Manager to join our growing team serving the Northampton and surrounding communities. • Comprehensive Home Health Training - No home health experience? No problem. We'll provide hands-on training, clinical support, and mentorship as you learn home health nursing, documentation, and OASIS.

As a Home Health RN Case Manager, you will manage a caseload of patients and provide skilled nursing care in the home environment while developing your clinical expertise in a growing and dynamic setting.

• Perform comprehensive patient assessments and develop individualized care plans

• Conduct initial and ongoing skilled nursing visits based on patient needs and physician orders

• Monitor and evaluate patient status, including physical, psychosocial, environmental, and caregiver factors

• Coordinate care with physicians, therapists, social workers, and other care team members

• Communicate changes in patient condition and recommend appropriate interventions

• Update and revise care plans as patient needs evolve

• Provide education and support to patients and caregivers

• Maintain accurate, timely clinical documentation in compliance with regulations and company standards

• Ensure adherence to HIPAA and patient confidentiality requirements

• Current, unrestricted Registered Nurse (RN) license in the applicable state

• Minimum of 1 year of nursing experience in a clinical setting

• Valid driver’s license, auto insurance, and reliable transportation

• Willingness to travel within the assigned territory (approximately 50%);

• Ability to sit, stand, bend, lift, and move intermittently

• Ability to lift 50–100 lbs as neededSign-On
