Schedule: Day-Shift | PRN | 8:00AM-4:30PM, weekday and weekend availability. 

Salary Range: $50.00 per hour

How you’ll make an impact in this role
  • Coordinate comprehensive care transitions by serving as the primary link between all clinical disciplines, ensuring every patient receives a seamless and well-integrated recovery experience.

  • Design individualized discharge pathways that integrate patient values and family preferences, ensuring a safe and sustainable transition from the hospital to the next stage of care.

  • Advocate for vital patient resources by identifying and removing systemic barriers to care, ensuring every individual has the medical necessity and support required for their specific diagnosis.

  • Evaluate clinical documentation accuracy to provide expert recommendations on levels of care, ensuring that medical records reflect the true complexity of the patient's journey and treatment options.

  • Drive continuous care improvement through active collaboration with the care transition team, constantly re-evaluating health plans to adapt to a patient’s changing condition in real-time.

What minimum requirements you’ll need

Licensure / Certification / Registration:

  • Certified BLS Provider specializing in Health Service Provider credentialed from the American Heart Association (AHA) obtained prior to hire date or job transfer date. Or military CPR-First Aid training required.
  • Licensed Registered Nurse credentialed from the Maryland Board of Nursing or current home state license for multi-state license recognition "Compact State" obtained prior to hire date or job transfer date required.

Education:

  • Preferred as of Mar15: Bachelor of Science Nursing 
  • Graduate of an accredited school of nursing required.

Work Experience:

  • Preferred as of Mar15: Two (2) years of Registered Nursing experience.
  • For IMCU 32022-14550: Telemetry experience.
What additional preferences we're seeking
  • Case Management Expertise: A minimum of 3 years of dedicated experience in a Case Management role within a healthcare setting.
  • Discharge Planning Expertise: Demonstrated experience in coordinating complex patient transitions and multi-disciplinary care plans.
