Job Title: Case Management RN Location: Eureka, CA Start Date: 06/08/2026 Duration: 13 weeks Shift: Days, 5x8

Requirements:

  • Minimum 2 years acute care Case Management experience
  • Acute care hospital experience required
  • Must be open to floating between like units based on need
  • Weekend requirement: Every Other Weekend
  • Experience with Epic charting preferred
  • No call required

Preferred:

  • Strong discharge planning and care coordination background
  • Ability to adapt to department needs
