A bit about this role: Transitions of Care (TOC) is one of Devoted Medical's largest clinical programs. From admission through 30 days post-discharge, our virtual interdisciplinary teams help thousands of patients get home safely and stay there, clearing the clinical, social, and logistical barriers that usually get in the way. Our Social Worker Case Managers are the advocates, coaches, and connectors in that work — building trusting relationships with patients and caregivers, developing individualized plans of care that align community, clinical, and financial resources, and supporting patients discharging from a behavioral health level of care. We're looking for a Manager of Social Work Case Management to build, lead, and inspire that team, along with our behavioral health care coordinators. You'll also serve on TOC's interdisciplinary management team alongside managers from our physician, NP/PA, RN case management, and care coordination functions, sharing accountability for the clinical, operational, and cultural success of our front-line care teams.

Responsibilities and Impact will include:
  • Building and developing a high-performing team of Social Worker Case Managers and behavioral health care coordinators, with a culture of service that shows up in patient outcomes.
  • Tracking staff performance through competency check-offs, audits, shadowing, and 1:1s, and coaching where support is needed.
  • Creating workflows, assessments, and plans of care that build efficiency for the team and measurable results for patients.
  • Supporting the team on escalated patient needs.
  • Leading onboarding for new teammates and ongoing education for existing staff.
  • Partnering with cross-functional teams to drive execution and standardize workflows across programs.
  • Meeting regularly with the Senior Manager of the RN and SW TOC functions to review team performance, wins, and areas for improvement.
  • Spend 30–40% of your time on direct patient outreach and case work, staying close to what your team navigates day to day and where the Social Work care model needs to improve.
Required skills and experience:
  • An active social work license (LMSW or higher).
  • 3+ years of experience in case management.
  • 3+ years of experience in people management, including developing and retaining a high-performing team.
  • Experience working with — and preferably managing — interdisciplinary care management teams.
  • Ability to independently manage a fully remote schedule, M–F, 8:30am–5:00pm CST, MST, or PST.
Desired skills and experience:
  • Experience leading clinical teams and operations with direct accountability for health outcomes, including driving new initiatives to measurable results.
  • Strong analytical instincts paired with strong clinical social work skills — you can set quantitative goals and act on data, and you're confident training a team to manage complex patients so needs don't fall through the cracks.
Salary range

$73,000 - $110,000 annually
The pay range listed for this position is the range the organization reasonably and in good faith expects to pay for this position at the time of the posting. Once the interview process begins, your talent partner will provide additional information on the compensation for the role, along with additional information on our total rewards package. The actual base salary offered will depend on a variety of factors, including the qualifications of the individual applicant for the position, years of relevant experience, specific and unique skills, level of education attained, certifications or other professional licenses held, and the location in which the applicant lives and/or from which they will be performing the job.
