MLR is thrilled to present an exciting opportunity for a dedicated and experienced Case Manager Registered Nurse (RN) to join our client's healthcare team near Rio Rancho, New Mexico!
Key Responsibilities for Case Manager RN:
  • Coordinate and oversee patient care plans to ensure continuity and quality of care.
  • Perform patient assessments, including psychosocial, medical, and functional evaluations.
  • Collaborate with interdisciplinary teams to develop individualized care plans and monitor patient progress.
  • Act as a liaison between patients, families, and healthcare providers, facilitating effective communication and resource coordination.
  • Advocate for patient needs while ensuring compliance with healthcare regulations and standards.
  • Maintain accurate documentation of patient care activities in accordance with facility policies and regulatory requirements.
