About Making Opportunity Count

Making Opportunity Count’s mission is to empower families to achieve economic security by eliminating barriers and creating opportunities. To alleviate poverty, we deliver a variety of services including childcare, education, workforce development, nutrition, health, and community and housing support.

Position Overview:

The Shelter Case Manager is responsible for providing comprehensive case management services to families residing in shelter. Using motivational interviewing and strength-based practices, the Case Manager works collaboratively with participants to develop individualized rehousing plans that include clear goals, measurable action steps, and targeted interventions designed to promote self-sufficiency and secure safe, stable, and affordable permanent housing.

Shelter Case Managers are cross-trained in the Stabilization Program and may be assigned to provide Stabilization services as needed to support program operations and ensure continuity of client services.

Essential Duties & Responsibilities:

  • Establish reoccurring monthly rehousing meetings with families.
  • Provide accurate written notices of meetings according to EOHLC Uniform Shelter Rules, Scope of Services, and program requirements.
  • Review and address barriers to housing (credit score, criminal record, history of nonpayment, setting up bank account).
  • Collect required vital records, financial documents, legal documents, and other necessary records to complete and maintain participant files. Supports participants with retrieving needed documents if necessary.
  • Review client preferences and needs for tenancy (location, roommate(s), etc.).
  • Create a budget to ensure housing sustainability (what can client afford vs. wants and needs).
  • Document case management efforts and services through clear, detailed, and thorough case notes in ETO for each family to include weekly meetings, client, and collaborative contacts, as well as any significant events and/or interactions within 24 hours of meeting.
  • Provide education about affordable housing options. Works collaboratively with other department staff.
  • Make referrals for support and services participants may need.
  • Updates weekly meeting reports, housing placements reports, and any other reports as required. Determine need and advocate for reasonable accommodations when needed.
  • Reports and updates of any program rule violations, missed meetings or concerns to appropriate leadership personnel to process noncompliance requests. Consistently meets all EOHLC Scope of Services and agency contract outcome goals with respect to housing efforts, housing placements, rehousing plans, and data collection.
  • Participate in all required training and professional development activities. Performs other duties as assigned.
  • Conduct home visits for participants receiving Stabilization services to assess housing stability, monitor progress toward goals, provide supportive case management, and connect families with community resources
  • Maintain accurate and timely documentation of all home visits and participant interactions in accordance with program and agency standards
  • When assigned, provide community-based case management to support participant engagement and housing stability.

Required Skills and Qualifications:

  • Associate's degree in human services, or related field.
  • 1 year of experience providing case management services focused on pre-tenancy and tenancy-sustaining services.
  • Must be empathetic to the needs of a diverse socio-economic population.
  • Strong interpersonal skills.
  • Knowledge of community resources, state and federal benefits and services.
  • Strong project management skills with the ability to administer multiple cases simultaneously and prioritize workload. Basic math and budgeting skills.
  • Ability to work effectively as a team member with other members of program and agency leaders.
  • Demonstrated prior success in case management functions or eligibility determinations for community-based social service organizations.
  • Problem-solving abilities.
  • Ability to communicate and collaborate with providers and community partners.
  • Strong internet, database, and computer skills.
  • Good documentation skills.

Physical Requirements:

  • Ability to move throughout the facility to conduct regular rounds and room checks, including at locations with stair-only access
  • Must be able to ascend and descend multiple flights of stairs regularly
  • Ability to move safely throughout worksites for extended periods of time during a shift
  • Ability to bend, kneel, crouch, and reach as needed to perform room inspections, support shelter operations, and respond to client needs
  • Ability to lift and carry up to 30 pounds
  • Must be able to perform tasks that involve physical movement, such as setting up rooms, moving light furniture, or supporting emergency protocols
  • Visual and auditory acuity sufficient to monitor client activity and ensure a safe shelter environment
  • Ability to respond quickly in emergencies, including the ability to move swiftly throughout the building

Compensation:

A salary within the range of $24.52 to $27.00 per hour will be provided to the successful candidate having met the minimum requirements for the position consistent with the MOC compensation philosophy.
