Case Management Assistant - Contingent - West Bloomfield

GENERAL SUMMARY:ย 

Assists the Case Managers with concrete patient-related tasks and various clinical interventions.ย 

The Case Manager Assistant assists patients, families, and others requesting assistance from the Case Management & Social Work Services Department but who are not currently receiving services from another staff member. The Case Manager Assistant may also be asked to perform other office-related tasks, e.g., assisting with special medication fund programs, updating resources, issuing Medicare Important Message to patients, etc.ย 

EDUCATION/EXPERIENCE REQUIRED:ย 

  • High School diploma or G.E.D. equivalent
  • Associates degree, preferred or equivalent healthcare experience
  • Good verbal and written communication skills
  • Ability to read, write and speak English
  • Good critical thinking and problems solving skills necessary
  • Good organizational skills
  • Ability to multitask. Good interpersonal skills, including ability to develop and maintain trusting relationships
  • Ability to conduct self in a professional manner
  • Ability to be flexible
  • Ability to complete all assigned tasks with little supervision
  • Ability to move freely about the facility in order to visit patients
