Join Our Team as an Assistant Case Manager!


Are you passionate about making a difference in the lives of individuals with special needs and their families? We are looking for a compassionate, organized, and motivated Assistant Case Manager to join our growing team!


This is a fast-paced and rewarding position that provides case management services to individuals with intellectual and developmental disabilities while coordinating with families, community providers, and state agencies. The ideal candidate will serve as the Director's Alternate Representative and play a key role in ensuring participants receive high-quality, person-centered services.


Key Responsibilities

  • Meet with participants face-to-face as required.
  • Conduct and participate in team meetings.
  • Maintain accurate, timely, and complete case documentation.
  • Assist individuals in accessing medical, behavioral health, educational, social, and community resources.
  • Develop, implement, and update Individual Plans of Care (IPCs) and methodologies based on each participant's unique needs.
  • Educate individuals and their families regarding available services and supports.
  • Advocate for participants while respecting their rights and dignity.
  • Represent the organization professionally and ethically in all interactions.
  • Interpret electronic monitoring messages and daily summary reports.
  • Utilize strong critical thinking, organizational, and problem-solving skills.

Qualifications

  • Proficiency with Microsoft Office and general computer applications.
  • Ability to work effectively with individuals from diverse cultural backgrounds.
  • Strong verbal and written communication skills.
  • Ability to remain calm and professional in stressful situations.
  • Ability to collaborate with community providers and build positive relationships with participants and families.
  • Dependable transportation, a valid Texas driver's license, and current automobile liability insurance are required.
  • Bilingual (English/Spanish) is preferred but not required.
