A professional registered nurse who plans, manages, coordinates, implements, and evaluates the ongoing care and psychosocial needs of the patient.

  • The case manager is responsible for ongoing monitoring and evaluation of patient outcomes and for the

  • On-call rotation for discharge planning after 5 pm and on weekends

    Qualifications
    MINIMUM QUALIFICATIONS:
    Current RN License in the state of Texas.
    Two (2) years clinical experience in hospital setting.
    BLS required.
    Prefer Utilization Review/Discharge Planning background
