Social Worker - Home Health Care - Macomb County Territory - 40 Hours/Week - Days

Romeo, Bruce Township, Washington, Utica, Shelby, Sterling Heights, Armada, Ray, Macomb, Richmond, New Haven/Lenox, New Baltimore, Chesterfield, Columbus, Casco, Fair Haven, Ira
Henry Ford Home Health Care Henry Ford Home Health Care is a comprehensive network of home health products and services that focuses on the complex needs of patients and caregivers. Available programs include chronic care management, joint replacement care, specialty infusion and transplant care, wound, ostomy and continence nursing, and palliative care to name a few. Henry Ford Home Health Care's services are provided by qualified registered nurses, licensed practical nurses, certified wound, ostomy and continence nurses, physical therapists, occupational therapists, speech - language pathologists, medical social workers, dietitians, and home health aides.
GENERAL SUMMARY:
Under general supervision, implements treatment programs to assist homebound clients and their families to adjust to personal and environmental difficulties, which predispose illness or interfere with obtaining maximum benefits from medical care.
EDUCATION REQUIRED:
  • Master's degree in social work required.

EXPERIENCE REQUIRED:
  • Requires a minimum of one (1) year of social work experience in a health care setting.

SKILLS REQUIRED:
  • Knowledge of the principles of human behavior, individual and group counseling techniques and relevant statutes and regulations.
  • Requires very high levels of social sensitivity, personal emotional adjustment and verbal skills in order to provide counseling, assist, and influence others.
  • Requires a professional level of analytical skills in order to solve problems, analyze, and develop policies and procedures.

CERTIFICATIONS/LICENSURES REQUIRED:
  • Requires Licensure in the State of Michigan as a Social Worker (LMSW).
  • Must possess a current State of Michigan driver's license and a satisfactory driving record.
