What is a care connection manager?

A Care Connection Manager is a healthcare professional who coordinates and oversees patient care and services, acting as a central point of contact between patients, families, and healthcare providers. Their primary role is to ensure that patients receive appropriate, timely, and seamless care by managing referrals, scheduling appointments, and facilitating communication among care teams. They also provide support, education, and resources to patients, helping them navigate complex healthcare systems. Care Connection Managers typically work in hospitals, clinics, or managed care organizations and play a crucial role in improving patient outcomes and satisfaction.

What are the key skills and qualifications needed to thrive as a care connection manager?

To thrive as a Care Connection Manager, you need a background in healthcare administration, case management, or social work, often supported by a relevant degree and experience coordinating patient care. Familiarity with care management software, electronic health records (EHRs), and sometimes certifications like CCM (Certified Case Manager) are typically required. Strong communication, organizational skills, and empathy are essential for building trust with patients and collaborating effectively with healthcare teams. These skills ensure seamless care coordination, improved patient outcomes, and efficient service delivery in complex healthcare environments.

What are some common challenges faced by care connection managers when coordinating care across multiple providers?

Job description

Midland Care Connection
Midland Care Connection is a not for profit, community based organization dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program. Our mission centers on "meeting individual needs through innovative care solutions," and we serve people across multiple counties in northeast Kansas.
Hospice House
A peaceful, 12 bed, inpatient facility dedicated to providing compassionate, end of life care when a home setting is no longer feasible. Here, patients receive around the clock, skilled hospice care from a multidisciplinary team-nurses, social workers, chaplains, and other specialists-focused on managing pain and symptoms with dignity and respect. The environment is designed to feel like home, offering comfort not just to the patients but also to their loved ones, providing space for visitation, emotional support, and moments of peace in the final chapter of life.
RN Hospice House
Provides hands on, comfort focused nursing care to patients receiving end of life services in an inpatient hospice setting. The role centers on assessing and managing symptoms (especially pain), administering medications, and ensuring patients remain as comfortable and dignified as possible. The nurse also updates care plans, documents all care provided, and works closely with the interdisciplinary hospice team (physicians, aides, social workers, chaplains, and volunteers). In addition, they support and educate families, helping them understand what to expect and offering emotional reassurance during a difficult time. Overall, the position combines clinical skill with compassionate, holistic care focused on quality of life rather than curative treatment.
Hours
Full Time 3-12 hour shifts, 7:00pm-7:00am
