SUMMARY:
Provides quality, supportive, compassionate care to assigned clients in their place of residence.Services are delivered according to the individualized service plan and, when applicable, under the delegation and supervision of a Registered Nurse.The HCA/CNA promotes client safety, dignity, independence, and quality of life while adhering to all agency policies, state regulations, and professional standards.
ESSENTIAL JOB FUNCTIONS / RESPONSIBILITIES:
Safety & Emergency
1. Attends annual safety and infection control education required by the agency. 2. Uses safety precautions as appropriate for patients, and self, e.g. proper positioning of self, good body mechanics, universal precautions, defensive driving skills.
3. Knows evacuation plan in each home that service is provided.
Standards
1. Support mission, values and goals of Signature Healthcare at Home.
2. Meets Signature Healthcare at Home standards for attendance and punctuality; maintains professional manners in dress and in speech.
3. Completes mandatory education requirements annually and documents same.
4. Follows agency policies.
5. Maintains confidentiality of clients' and families' private health and other information per HIPAA.
6. Attends agency staff meetings as required.
7. Functions as a member of a team of caregivers, promoting team communication and support.
8. Meets productivity expectations for this position; organizes tasks and manages time effectively
9. Documents client services legibly, accurately and in concert with agency standards.
10. Can work well alone, without supervision. Asks appropriate questions when unsure of an assignment, takes initiative in areas expected of him/her. Uses good judgment in difficult situations.
11. Works as scheduled notifies the supervisor or designee of delays or absences within established expectations.
12. Able to solve problems effectively and report accordingly when the home environment is not physically safe for staff or the client.
13. Adheres to agency policies, code of conduct, and ethical standards.
Work Performed
1.Assist clients with activities of daily living (ADL's), personal hygiene including shampooing hair, grooming, bathing, shaving, dressing, mouth care, transfers, toileting, and ambulation as instructed by the service plan.
2.Performance of simple household and supportive services such as light housekeeping related to client care, including laundry, linen changes, and tidying living areas. Assist with meal preparation and feeding as directed. Support shopping, errands and appointment reminders.
3.As permitted by state law and agency policies; perform delegated nursing tasks under RN delegation, which may include vital signs, assistance with self-administered medications, blood glucose checks, and selected ostomy, catheter, or wound care when delegated and trained.
4. Complete all documentation on the same day services are provided, following agency standards
5.Follow infection control, universal precautions, and safety protocols always.
6.Any other task the agency may choose to have the home care aide perform according to state specific regulations.
Required:
  • Washington State Home Care Aide (HCA) certification or active HCA training status within the required timeframe.
  • OR current Washington State Certified Nursing Assistant (CNA) license in good standing.
  • Current CPR and First Aid certification, or willing to obtain.
  • Food Handlers permit, or willing to obtain.
  • Ability to pass background check and meet state licensing requirements.
  • Ability to pass drug testing.
  • Reliable transportation and valid driver's license.

Preferred:
  • Prior experience in home care, home health, or senior living.
  • Experience with dementia or chronic conditions.
  • Strong interpersonal and communication skills.
