What are some common challenges faced by Cardiac Anesthesiologists during complex heart surgeries?

Cardiac Anesthesiologists often face the challenge of managing rapid physiological changes in patients during complex heart surgeries, such as open-heart procedures or heart transplants. They must closely monitor and maintain the patient's hemodynamic stability and be prepared to respond swiftly to complications like arrhythmias or severe blood loss. Additionally, they work closely with cardiac surgeons, perfusionists, and nursing staff to coordinate care, requiring exceptional communication and teamwork skills. The high-stakes environment demands not only technical expertise but also the ability to remain calm and decisive under pressure.

What are the key skills and qualifications needed to thrive as a Cardiac Anesthesiologist, and why are they important?

To thrive as a Cardiac Anesthesiologist, you need extensive medical training in anesthesiology, cardiac physiology, and perioperative care, usually supported by a medical degree, anesthesiology residency, and a cardiac anesthesia fellowship. Familiarity with advanced monitoring systems, transesophageal echocardiography (TEE), and certification from boards like the American Board of Anesthesiology are critical. Exceptional teamwork, decision-making under pressure, and strong communication skills distinguish top professionals in this role. These competencies ensure patient safety, optimal surgical outcomes, and effective collaboration in high-stakes cardiac procedures.

What are Cardiac Anesthesiologists?

Cardiac anesthesiologists are medical doctors who specialize in providing anesthesia care to patients undergoing heart surgery and other cardiac procedures. They play a critical role in managing the patient’s pain, vital functions, and overall safety before, during, and after operations involving the heart. These specialists are also trained to handle complex cardiovascular conditions and often work closely with cardiac surgeons and other healthcare professionals to ensure the best outcomes for patients. Their expertise includes advanced monitoring, life support techniques, and the use of specialized medications tailored to cardiac cases.

What is the difference between Cardiac Anesthesiologist vs Thoracic Anesthesiologist?

While both Cardiac Anesthesiologists and Thoracic Anesthesiologists specialize in anesthesia for specific surgical areas, the main difference lies in their focus: cardiac anesthesiologists concentrate on heart surgeries, whereas thoracic anesthesiologists handle lung and chest procedures. Both roles require similar credentials and work in specialized surgical environments, but their expertise is tailored to different parts of the thoracic and cardiovascular systems.

What Does a Cardiac Anesthesiologist Do?

As a cardiac anesthesiologist, you study anesthesia medicines and how they affect the body’s organs. Your responsibilities are to administer anesthesia to a patient, monitor their vital signs, and maintain a safe level of anesthetics during heart surgery. You meet with each patient beforehand, asking them about medications and health problems to ensure the safety of their operation. Other duties include working with advanced technology to monitor and regulate organ systems, as well as monitoring postoperative progress. As a cardiac anesthesiologist, you primarily work during heart surgery.

Job description

Choose Your Cases. Shape Your Schedule. Practice with Autonomy.
Location: Rochester, NY (multiple hospitals and surgical centers nearby)
Organization: Rochester Regional Health
Position Type: Full-Time
Cardiac Anesthesiologist
At Rochester Regional Health, our anesthesiologists provide comprehensive perioperative care across a wide range of surgical specialties in a collaborative, physician-led environment. We are seeking a fellowship-trained Cardiac Anesthesiologist to join our growing and highly specialized cardiac program.
This position offers substantial cardiac case exposure, flexibility in case selection, and a supportive call structure designed with long-term sustainability in mind. Physicians benefit from a practice model that combines the autonomy and flexibility often associated with private practice with the stability of an integrated health system.
Position Summary
The Cardiac Anesthesiologist provides high-quality anesthesia care for patients undergoing cardiac, thoracic, vascular, and general surgical procedures. Physicians practice at the top of their training in both personally performed and supervisory care models, with flexibility in case selection through our physician-driven scheduling system.
Cardiac anesthesiologists typically spend approximately 60-70% of their clinical time performing cardiac and thoracic anesthesia, while maintaining the option to participate in general anesthesia cases based on individual preference and departmental needs.
Clinical Responsibilities
Provide anesthesia care for complex cardiac surgical procedures, including:
  • Coronary artery bypass grafting (CABG)
  • Valve repair and replacement
  • Left ventricular assist device (LVAD) placement
  • Minimally invasive valve and CABG procedures
Support structural heart and hybrid procedures including:
  • Transcatheter Aortic Valve Replacement (TAVR)
  • MitraClip
  • Watchman procedures
Additional responsibilities include:
  • Providing anesthesia for the majority of thoracic procedures
  • Supporting high-complexity vascular cases
  • Participating in a predominantly personally performed cardiac anesthesia model
  • Collaborating with highly experienced cardiac CRNAs as the department continues to thoughtfully expand its cardiac care team model
General Anesthesia Responsibilities
  • Provide anesthesia services across a broad range of surgical specialties
  • Participate in daily case assignments through an equitable physician pick system that allows flexibility to select either cardiac or general assignments
  • Supervise CRNAs and/or personally perform cases as appropriate
  • Deliver comprehensive perioperative care including preoperative evaluation, intraoperative management, and postoperative assessment
The physician scheduling model offers a unique level of autonomy and flexibility rarely found in employed practice settings.
Call Structure
We recognize the importance of balancing subspecialty practice with a manageable and sustainable call model.
Cardiac Call
  • One weekday cardiac anesthesia call per week (from home)
  • One cardiac anesthesia weekend call approximately every 5-8 weeks (from home)
  • Heart call activation rate is approximately 10%
Because heart call is home-based with a low activation rate, post-call days are not routinely scheduled. However, physicians who are called in overnight are prioritized for relief as early as possible the following day.
General Overnight Call
  • Cardiothoracic anesthesiologists take reduced in-house overnight general anesthesia call responsibilities - approximately once per month - in exchange for participation in home cardiac call
  • In-house overnight calls are general anesthesia calls and do not include cardiac anesthesia responsibilities
  • Guaranteed post-call day off following overnight in-house call shifts
Why Physicians Choose Rochester Regional Health
  • Physician-led department with collaborative leadership and meaningful physician input
  • Flexible scheduling model with autonomy in case selection
  • Stable and growing cardiac program with strong institutional support
  • Collegial culture focused on teamwork, quality, and long-term sustainability
  • No non-compete clauses or exclusivity requirements
  • Integrated health system with a strong reputation for community-based care
Life in Rochester, NY
Rochester offers the advantages of a mid-sized city with easy access to the Finger Lakes, outdoor recreation, excellent public schools, and a vibrant arts and food scene. Physicians enjoy short commutes, affordable living, and proximity to all major hospitals and surgical centers.
Qualifications
  • Board Certified or Board Eligible in Anesthesiology
  • Completion of an ACGME-accredited Cardiac Anesthesia Fellowship
  • Certification in Advanced Perioperative Transesophageal Echocardiography (TEE)
  • Eligible for medical licensure in New York State
