Key Responsibilities

  • Develop and execute discharge plans that link patients with appropriate community resources, ensuring smooth transitions from care.

Patient Population / Client Focus

Qualifications
  • A Master's degree in Social Work from an accredited program.
  • Active state licensure as a Licensed Clinical Social Worker (LCSW) in Illinois.
  • Proficiency in social work practices, particularly within healthcare settings.
