• Conduct detailed psychosocial assessments to identify the emotional, social, and financial needs of patients.
• Provide individual and group counseling tailored to the unique challenges faced by our patients, including issues related to adjustment, grief, and coping.
• Collaborate with medical personnel to devise comprehensive treatment plans that address both physical and psychosocial needs.
• Coordinate with community organizations to ensure that patients have access to necessary post-discharge resources and support services.
• Participate actively in hospital committees, offering insights on social work practices that enhance patient care and support.
• Develop and execute discharge plans that link patients with appropriate community resources, ensuring smooth transitions from care.
• Maintain accurate and timely documentation in compliance with hospital policies and legal requirements, enhancing overall operational efficiency.
• Engage in crisis intervention as necessary, providing immediate support to patients and their families during difficult times.

The focus of your work will be primarily with patients at the Hospice Center, helping them and their families cope with the emotional and psychological aspects associated with terminal illnesses. You will offer sensitive, empathetic support tailored to individual needs, providing critical guidance and comfort as they navigate the complexities of end-of-life care.

• A Master's degree in Social Work from an accredited program.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) in Illinois.
• Proficiency in social work practices, particularly within healthcare settings.

• At least two years of experience working in a healthcare environment, preferably in a social work capacity within a hospital or hospice setting.
• Strong knowledge of medical terminology and a solid understanding of mental health disorders and crisis intervention techniques.
• Demonstrated ability to communicate effectively with patients, families, and healthcare team members.
