Job description

  • Position Summary
    • Under the general supervision of the Manager of Specialty Services, the Resource Coordinator is responsible for the facilitating, coordinating in conjunction with health care teams, staffing, supplies/equipment, and patient flow/scheduling. Utilizes the nursing process to care for pediatric, adolescent, adult, and geriatric patients based on current standards of practice. Functions within the permissible scope of nursing practice as defined by the Georgia Board of Nursing and St. Joseph's/Candler Health System policies and procedures. Maintains an open communication and is available to component organizations and practitioner sites for consultation.

    • 1-2 Years - Required
    • Must have the ability to work effectively with fellow employees. MDs, and patients, as well as the general public. Developing, motivation, assessing, and negotiating are required in positions which causes change or to obtain agreements to enhance organizational effectiveness.
  • License & Certification
    • Registered Nurse License with State of Practice – Required
    • National Certification - Preferred
    • Graduation from an approved school of nursing and current registered nurse license to practice in the State of Georgia.  Wound care education and debridement certification required.
    • Basic Life Support (BLS) - Required
  • Core Job Functions
    • Demonstrates knowledge and skills to provide care appropriate to any age-related needs of the patient being served. Coordinates patient care needs with team members and other appropriate departments. Coordinates/oversees patient placement within the system and monitors house availability and assesses patient needs for house placement.
    • Contributes to formulation of nursing careplan for patients in consultation with the RN and in collaboration with patient, families, healthcare members using established nursing diagnoses and nursing protocols. Prioritize nursing care needs of clients. Assist in the review and revision of nursing care plans with the RN to meet the changing needs of the patient.
    • Assess data related to basic, physical, developmental, spiritual, cultural, functional, and psychosocial needs of the patient. Collects data within established protocols and guidelines from various sources. Documents data collection, assessment, and communicates findings to appropriate healthcare members.
    • Assessment and reassessment are completed and changes in status are communicated to appropriate care provider. Plan of care is formulated and modified based on evaluation. Collaborates with disciplinary health care team to improve patient outcomes.
    • Assist in forming teams for process improvement. Proposes new and innovative approaches for dealing with patient flow. Conduct daily huddles to coordinate the admissions and transfer of patients.
    • Responds to patient requests and proactively addresses patient needs using a multi and interdisciplinary approach. Works with the unit team to address patient satisfaction scores and works with unit leadership to develop and implement action plans.
    • Demonstrates a consistent level of performance. Strives to maintain a steady level of productivity. Monitors staff's nursing care and other assigned duties to assure optimal care is given and reports any problem.
