About the Role:
We are seeking a compassionate, mission-driven, and clinically skilled Social Worker (LMSW) to join our Behavioral Health team in a grant-funded role supporting two specialized programs: Hook Up and Bridge (HUB) and Crystal Methamphetamine Harm Reduction Services (MTH/MTC).
The Social Worker provides individual psychotherapy to Callen-Lorde patients and members of their families of choice, as well as family/couple and group psychotherapy. This role also includes conducting behavioral health assessments for medical patients, providing crisis intervention, supporting care coordination and case management needs, and referring patients within Callen-Lorde or externally as appropriate.
This position is focused on the clinical and programmatic deliverables of the HUB and MTH/MTC grants. Services delivered, caseload, work hours, and site assignments will be guided by the requirements of these grant-funded programs and may shift based on operational and program needs.
Key Responsibilities
  • Provide comprehensive behavioral health services within LMSW/LCSW scope of practice
  • Conduct assessments and facilitate appropriate referrals
  • Deliver crisis intervention services when needed
  • Provide individual, family/couples, and group psychotherapy
  • Collaborate with behavioral health management, psychiatry providers, primary care providers, and care coordinators
  • Maintain current productivity standards through patient scheduling and monitoring
  • Participate in program development and quality improvement initiatives
  • Support grant-related program goals, service delivery requirements, and documentation/reporting expectations as assigned
  • Write letters for patients seeking gender-affirming surgery as appropriate
  • Participate in agency on-call services

Work Schedule:
This is a full-time position based within Callen-Lorde's Behavioral Health Clinic, which operates Monday through Friday. The role requires onsite presence four (4) days per week, with one (1) hybrid/remote day, and includes some evening hours based on clinic and patient care needs.

  • Master's degree in Social Work and NYS licensure - LMSW or LCSW required
  • 1+ years post-Master's clinical experience working with both adult individuals and groups preferred
  • Experience working in New York StateArticle 31 and/or Article 28outpatient practice setting preferred
  • Knowledge of LGBT- and HIV-related health/mental health issues and concerns
  • Excellent interpersonal, written, and oral communication skills
  • Computer literacy is required.
  • The ability to maintain ongoing documentation in an electronic charting system (EPIC)
  • Bilingual (Spanish/English) preferred
  • Commitment to the mission of Callen-Lorde Community Health Center
  • Ability to work with diverse populations, and ability to handle multiple, simultaneous tasks.
