Job description

Coordinates the transfer, transportation, and acceptance of patients for other facilities. Collects clinical information relative to a transfer request and then facilitates the transfer by matching the patients’ needs to the facility with bed capacity and service capability and the discretion of attending and nursing staff.

  • Works in collaboration with multiple departments to foster relationships with referring facilities, physicians, and hospital staff;
  • Performs transfer call center functions in a responsive manner answering calls, entering into a database consisting of facility service profiles, approved clinical protocols, established patient care protocols and healthcare information;
  • Coordinates transport of emergent, urgent and elective patients to a final destination facility from all potential referral sources to include but not limited to hospitals, clinics, nursing homes and physician offices.
  • Facilitates patients throughput to a final destination with an ongoing focus on quality and efficiency;
  • Works directly with others involved in patient flow to ensure timely transfer, appropriate bed assignment, and provides accurate patient information to assist with the development on an initial plan of care;
  • Ensures accurate and concurrent data collection and maintenance using Microsoft Excel or other software programs;
  • Facilitates patient flow thorough the continuum of care to ensure appropriate level of care is assessed based on clinical staff discretion;
  • Collects patient information needed to begin the transfer process from nursing or physician staff and relays information to potential receiving facilities.
  • Monitors self-compliance and implements process changes to ensure compliance to such regulations and quality initiatives;
  • Communicates with physicians and outlying facilities regarding all patient transfers, potential bed placements, and assists with coordinating patient movement from one facility to another;

      Connects nurse or physician with receiving facility physician or other staff to discuss patient condition and transfer requirements;

  • Ensures compliance with EMTALA when arranging transfer and transport with receiving facility at the direction of nursing staff and physician staff;
  • Related duties as required and other duties as assigned.
