Clinical Liaison – Skilled Nursing / Call Center Support

Aston Health is seeking a Clinical Liaison to join our team in a fast paced, high volume support role focused on hospital referrals, admissions coordination, and census development for our skilled nursing facilities.

This position works closely with hospitals, case managers, facility leadership, and internal teams to help coordinate admissions, review referrals, and support census growth across multiple locations. The ideal candidate is organized, responsive, clinically knowledgeable, and comfortable working in a call center style environment with frequent communication and multitasking.

Responsibilities

• Review and coordinate incoming referrals for skilled nursing facilities
• Communicate with hospitals, discharge planners, case managers, and facility teams regarding admissions status
• Verify clinical appropriateness and gather supporting documentation as needed
• Track referral activity, admissions, denials, and follow up items in real time
• Maintain strong communication with Admissions, Nursing, and Operations teams
• Support census development initiatives and assist with referral management processes
• Escalate barriers or concerns that may impact admissions or transitions of care
• Maintain accurate documentation and timely updates within internal systems
• Assist with insurance verification and authorization follow up when applicable

Qualifications

• Clinical background preferred, including LPN, RN, CMA, or healthcare admissions experience
• Previous skilled nursing, hospital, rehab, or post acute experience strongly preferred
• Prior call center, admissions, intake, or referral coordination experience preferred
• Strong communication and customer service skills
• Ability to multitask and work efficiently in a high volume environment
• Comfortable working with multiple systems, spreadsheets, and electronic documentation
• Strong organizational skills and attention to detail
