Liquid Personnel is recruiting anย Adult Social Worker to join its client's CHC team, based in Somerset. This is a key opportunity to contribute to high-quality Continuing Healthcare (CHC) assessments and support robust, evidence-based decision making within a multi-disciplinary environment.

What will your responsibilities be?ย 

  • Complete comprehensive CHC assessments across a range of care settings in line with the National Framework.
  • Lead and contribute to Decision Support Tool (DST) processes with accuracy and sound professional judgement.
  • Act as a core member of the multi-disciplinary team, representing the local authority perspective.
  • Provide constructive challenge to ensure consistent and lawful CHC eligibility decisions.
  • Apply in-depth knowledge of the Care Act 2014, including Section 22 and the limits of local authority provision.
  • Explore and support appropriate joint funding arrangements where applicable.

Qualifications and Experience:ย 

To be successful in this role you must have; ย 

  • Qualified Social Worker with current professional registration.
  • Extensive experience completing CHC assessments and DSTs.
  • Strong understanding of the CHC National Framework.
  • Detailed knowledge of the Care Act 2014, particularly Section 22.
  • Excellent communication, analytical and stakeholder management skills.
  • Ability to work effectively within a multi-disciplinary team.ย
