Job description

About Seen Health
At Seen Health, we are revolutionizing the way senior care is delivered through the PACE (Programs of All-Inclusive Care for the Elderly) model. Backed by top VCs, Seen Health is a culturally-focused, technology-enabled healthcare organization that integrates comprehensive medical care and social support with a high-touch, interdisciplinary approach.
Our mission is to empower seniors to age-in-place with dignity and provide their families peace of mind. We are building upon a proven Home and community based services model to create a culturally-competent and scalable PACE program. We are also building a comprehensive operating system focused on data and workflows that span across systems, processes, people, and care contexts. We want to empower our clinicians and staff with tools that deliver relevant data at the time and site of care and enable them to deliver exceptional care to our participants, which improve clinical outcomes, participant & provider satisfaction, and ultimately our strength as an organization.
We are a mission-driven, multidisciplinary team with deep healthcare, technology, and operations expertise, each inspired by our own personal stories of caring for seniors in our lives. Our name, Seen Health, was chosen to reflect our commitment to provide the highest standard of care to underserved older adults while respecting and incorporating their individual beliefs, heritage, and values, so that they can truly be seen.
About the Role
At Seen Health, Expansion is the engine that allows us to bring our transformative PACE model to more seniors. Our growth depends on our ability to architect complex, sustainable partnerships that allow us to enter and scale in new markets with speed and local resonance.
Reporting directly to the Head of Expansion, you will be a key driver of new market entry. This is a high-ownership, externally-facing role for someone who thrives building relationships, representing an organization with confidence, and getting things done without a lot of hand-holding. You'll work across two interconnected areas: local business development in the San Gabriel Valley and greater LA area, and new market expansion research and early relationship-building across emerging PACE markets nationally. This is not a back-office role. You'll be at community meetings, on the phone with health officials and prospective partners, and attending conferences. If you love building from the ground up and want to see your work have a direct impact on how we grow - this is the job.
Key Responsibilities
  • Local Business Development & Community Partnerships
    • Represent Seen Health at local coalition meetings, chamber of commerce events, cultural community gatherings, and senior care forums in the SGV and greater LA area
    • Build and maintain relationships with day health programs, community health centers, hospitals, senior centers, and local referral partners
    • Initiate and manage outreach to prospective care delivery partners - Mandarin-language outreach is a core part of this work
    • Identify and develop local BD opportunities that the team has mapped but hasn't had bandwidth to pursue
    • Help grow our referral and enrollment pipeline through consistent local presence
    Market Expansion & Intelligence
    • Monitor PACE program developments nationally - RFP timelines, legislative activity, regulatory readiness, and competitive landscape - across a portfolio of target states
    • Research new markets: population demographics, existing care providers, state agency contacts, and potential partnership opportunities
    • Contribute to application and proposal logistics as opportunities take shape
    • Surface timely opportunities to the strategy team and maintain organized, up-to-date market intelligence
Who You Are
Must-have qualifications:
  • Based in the Los Angeles / San Gabriel Valley area or willing to be
  • 2-5 years of experience in healthcare, community health, or a related field
  • Demonstrated ability to represent an organization confidently and effectively in meetings and partner conversations - you're the face in the room when leadership isn't there
  • Execution-oriented: you get things done with limited direction and follow up without being asked
  • Comfortable operating in a fast-moving environment where priorities evolve
  • Strong written communication for briefings, partner summaries, and internal updates
  • Mandarin fluency (spoken and written) - essential given the communities we serve and the nature of local outreach

Nice-to-have qualifications:
  • Experience with PACE, ADHC, FQHC, or other senior care / long-term care settings
  • Familiarity with DHCS, CMS, or state Medicaid program structures
  • Additional language fluency desired (Cantonese, Korean, or Spanish)
  • Prior experience supporting RFP responses or grant applications
  • Existing relationships in the SGV senior care or community health ecosystem

Salary & Benefits
  • Target salary range $90-$130K & competitive equity package, depending on experience
  • Benefits: Seen Health is proud to offer a robust benefits offering for our employees. In addition to traditional healthcare coverage, we also offer additional benefits to help further your wellness and feeling of being part of the team.
    • Medical, Dental, and Vision benefits for you and your family
    • Life Insurance and Disability Benefits
    • Parental and Caregiver Leave
    • Free Lunch, as well as delicious snacks and coffee to keep you energized
    • Paid Time Off across holidays, vacation time, personal days, and sick days
    • 401k Plan
    • Personal and professional development, including CME support and career growth opportunities
    • Subscriptions and training on using AI tools including ChatGPT
