Position Summary
The Community Patient Navigator is responsible for promoting Doral Health & Wellness services throughout the Hope Gardens and Bushwick communities while generating referrals for the organization. This position serves as a trusted community representative, building relationships with residents, healthcare providers, community organizations, churches, senior housing developments, and local businesses to identify individuals who may benefit from Doral Health & Wellness services.
The Community Patient Navigator educates prospective participants about available programs, assists with service connections and enrollment, and supports organizational growth through consistent referral development and community engagement.
What You'll Do
• Generate daily referrals for Doral Health & Wellness services.
• Conduct community outreach throughout Hope Gardens and Bushwick.
• Build and maintain relationships with healthcare providers, community organizations, churches, senior centers, housing developments, and local businesses.
• Promote Doral Health & Wellness programs through community events, health fairs, presentations, and outreach activities.
• Educate individuals and families about available healthcare and supportive services.
• Assist prospective participants with connecting to services and completing enrollment processes.
• Follow up on referrals to support successful enrollment and participation.
• Maintain accurate records of referrals, outreach activities, and community contacts.
• Submit daily, weekly, and monthly activity reports.
• Represent Doral Health & Wellness professionally at all times.

Who We're Looking For
Required Qualifications
• High School Diploma or GED required.
• Strong communication and interpersonal skills.
• Ability to work independently and meet performance goals.
• Comfortable working in a community-based environment.
• Bilingual (English/Spanish) required.
Preferred Qualifications
• Experience in community outreach, healthcare, social services, patient advocacy, sales, marketing, customer service, care management, or business development.
• Familiarity with the Hope Gardens and Bushwick communities strongly preferred.
• Existing community relationships and local connections are a significant plus.
• Experience working with Medicaid, Medicare, senior populations, or underserved communities is highly desirable.
