Registered Nurse (RN)

Overview: Bushwick Center is seeking a Registered Nurse (RN) to join our dedicated team at our Adult Day Care. This role is focused on delivering high-quality clinical assessments, care planning, and ongoing support for residents through the New York State Uniform Assessment System (UAS-NY).

As part of our interdisciplinary care team, the ideal candidate will be a compassionate, detail-oriented professional who is experienced in conducting in-person assessments, developing individualized care plans, and working closely with residents, families, and healthcare providers to promote resident well-being and regulatory compliance.

Duties:

  • Perform thorough in-person clinical evaluations for existing residents according to UAS standards, agency policies, and state laws.

  • Create individualized service plans and determine eligibility for ongoing participation through detailed UAS assessments.

  • Evaluate potential new residents by considering clinical, social, and environmental factors to establish appropriate levels of care ensuring safety and well-being.

  • Gather and analyze clinical information from multiple sources to support patient care decisions based on clinical guidelines and professional judgment.

  • Maintain clear and effective communication with residents, family members, healthcare providers, and other stakeholders to guarantee assessment accuracy.

  • Apply a strong understanding of the challenges faced by vulnerable populations with chronic or complex bio-psychosocial needs.

  • Ensure all assessments meet regulatory and organizational requirements.

Requirements:

  • Valid Registered Nurse (RN) license.

  • UAS-NY Certification required

  • Experience or familiarity with UAS-NY strongly preferred.

  • Excellent communication, organizational, and interpersonal abilities.

  • Self-motivated with the capability to work independently.

  • Proficient computer skills and fluent English communication.

  • Prior experience in MLTC or Managed Medicaid environments is a plus.
