This position operates in a remote work setting, allowing the LCSW to engage with clients from the comfort of their home. While the work is conducted virtually, our staff remains closely connected through regular virtual meetings and collaborative online platforms. As part of the hospice center continuum of care, the LCSW will have the opportunity to provide compassionate, comprehensive support to clients facing serious illnesses, emphasizing the importance of mental and emotional health during critical times.

• Provide direct clinical services to clients, offering therapy and support for individuals experiencing symptoms of major mental illness or severe emotional disorders.
• Collaborate as an integral member of a multidisciplinary team, contributing to shared goals and promoting a cohesive therapeutic approach.
• Conduct brief screenings that are evidence-based and developmentally appropriate, facilitating comprehensive assessments when necessary.
• Create, implement, and monitor integrated care plans that link clients, their families, and other healthcare providers, ensuring coordinated support and effective communication.
• Maintain a manageable caseload through individual, group, and family therapy sessions, ensuring a client-focused approach tailored to their unique needs.
• Regularly document client progress and treatment outcomes, providing timely reports and data submissions for ongoing assessment of care efficacy.
• Participate in on-call coverage outside of regular program hours as required to provide crisis intervention and support.
• Engage in quality improvement initiatives geared towards enhancing service delivery and optimizing client satisfaction within the organization.

The Licensed Clinical Social Worker will primarily serve clients at a hospice center, addressing the mental health needs of individuals who are dealing with life-limiting illnesses such as cancer, heart disease, and neurological disorders. This role is pivotal in providing emotional support to clients and their families during some of the most challenging times of their lives. The ability to establish rapport and cultivate trust is crucial, as many of the clients may be grappling with complex feelings, including anxiety, depression, and grief.

• A Bachelor’s degree in Social Work (BSW) and a current, valid state license as a Clinical Social Worker (LCSW).
• A minimum of 2 years of direct clinical experience, preferably in a healthcare or hospice setting, working with diverse populations.
• Demonstrated understanding of the complexities of mental health disorders and the associated therapeutic techniques.
• Excellent communication, interpersonal, and organizational skills.
• Ability to work effectively in a remote environment, maintaining productivity and connectedness with team members.

Experience in hospice care or palliative settings, knowledge of cultural competency, and previous involvement in integrated care models will be highly regarded. Familiarity with electronic health records (EHR) systems and previous participation in quality improvement projects are advantageous.

The compensation for this role ranges between USD 70,000 and 85,000 annually based on experience and qualifications.
