Concierge Nurse Advocate & Client Partnerships (Chicago based launching in Indiana) - Hybrid

As Broad Street expands from Chicago into Indianapolis, we are seeking an experienced, relationship-oriented nurse to help establish and grow our presence locally. Broad Street Advocates & Private Nursing provides concierge nursing, patient advocacy, and clinical care coordination services for individuals and families navigating complex health situations while remaining safely and comfortably in their homes.

Unlike traditional healthcare models, our private-pay concierge approach allows us to provide highly personalized, proactive support focused entirely on the client and family โ€” not insurance limitations, productivity metrics, or episodic care. We combine clinical expertise, advocacy,
and relationship-driven guidance to help families navigate healthcare with confidence and continuity.

The Opportunity:

This is a unique opportunity for a clinically experienced nurse who wants to move beyond traditional institutional healthcare and apply their expertise in a more autonomous, personalized, and impactful way.

You will serve as both a trusted clinical advisor to clients and families, while also helping introduce Broad Streetโ€™s model of care throughout the Indianapolis healthcare community.

Working closely with Broad Streetโ€™s leadership team in Chicago, you will help build and strengthen referral relationships with physicians, specialists, discharge planners, rehabilitation providers, senior living communities, and other strategic healthcare partners across the Indianapolis market.

The Role:

As an Indianapolis Nurse Advocate & Client Partnerships Manager, you will:

  • Guide families through complex healthcare situations and care decisions
  • Coordinate communication across physicians, specialists, therapists, and care teams
  • Provide proactive clinical oversight and advocacy for clients in the home setting
  • Serve as a trusted resource and connector for families navigating changing care needs
  • Collaborate with private nurses and caregivers supporting clients in the home
  • Build and maintain relationships with referral partners and healthcare professionals
  • Develop new client opportunities through strategic outreach and community engagement
  • Represent Broad Street Advocates within the Indianapolis healthcare ecosystem
  • Help establish and shape Broad Streetโ€™s concierge care model locally

This is not a traditional bedside nursing role or standard home care position. It is designed for clinicians who thrive in relationship-driven environments and want to apply their experience in a broader, more meaningful way.

Broad Street provides One-on-one Private Nursing in the home. Because we are private pay, we focus on a Concierge Level of service where we can bridge the gap between the healthcare system and home, unencumbered by the administrative burden of coding.
What does Private Nursing at Broad Street look like?
Completely Flexible Schedule and Location (What works for you) โ€“ Private Nursing Care Needs in the Home vary. As a result, we present the opportunities that work for you. This can include set schedules with long shifts, or shorter shifts. It can be a supplement to another job, or even PRN. We operate throughout Chicagoland. Itโ€™s really up to you.
No coding or administrative overhang โ€“ We are completely private pay. No more tedious Insurance documentation. We do documentation, but only as it relates to support of the patient and to each other.
Collaboration with other Nurses โ€“ All of our Cases involve Nurse Care Managers and Patient Advocates for support and collaboration. It is truly a team effort.
Opportunity for advancement โ€“ Virtually all of our Patient Advocates and Care Managers started as Private Nurses. For the right person, the sky is the limit.
