Home Health Registered Nurse (RN) | Pedia with Trach and Vent Experience | Morning Shift
BrightStar Care is seeking an experienced Registered Nurse (RN) to serve as a key clinical team member delivering high-quality care throughout Cuyahoga, Lorain, and Medina counties. This role supports new client admissions, urgent staffing needs, skilled nursing visits, and ongoing case management.

Schedule
  • Monday to Sunday: 8am -4pm
  • Fulltime hours which is 40 hours per week
  • Flexible scheduling based on patient needs

Job Responsibilities
  • Infant pediatric care
  • Seizure monitoring
  • Total assistance with activities of daily living (ADLs), including:
    • Bathing
    • Dressing
    • Feeding
    • Toileting
  • Medication administration
  • Mobility assistance and transfers
Skilled Nursing Care
  • Perform initial assessments, reassessments, and safety evaluations
  • Deliver skilled nursing services including wound care, injections, infusions, and other treatments within RN scope of practice
  • Monitor patient conditions and communicate changes appropriately
  • Maintain accurate and timely clinical documentation
  • Follow infection control and safety protocols
Case Management & Care Coordination
  • Develop and maintain individualized Plans of Care
  • Coordinate care with physicians, families, and interdisciplinary teams
  • Support client admissions and transitions of care
  • Ensure compliance with agency, state, federal, and Joint Commission standards
Staff Support & Supervision
  • Conduct supervisory visits and competency assessments
  • Provide clinical guidance and education to caregivers and nursing staff
  • Assist with orientation, skills validation, and ongoing training
  • Support quality assurance and chart review activities
Qualifications
Required
  • Current unrestricted Ohio Registered Nurse (RN) license
  • Graduate of an accredited nursing program
  • Minimum three (3) years of clinical nursing experience
  • Minimum one (1) year of case management experience
  • Current CPR certification
  • Valid driver's license, reliable transportation, and auto insurance
  • Ability to travel throughout assigned service areas
  • Ability to pass background check, drug screening, and other pre-employment requirements
Preferred
  • Home health experience
  • Healthcare management experience
  • Pediatric nursing experience
  • Experience supervising clinical staff
Physical Requirements
  • Ability to stand, walk, bend, kneel, reach, push, and pull as required
  • Ability to lift up to 50 pounds with or without accommodation
