Assessment Home Care Nurse

Position: Assessment Nurse (RN) Key Responsibilities:

  • Provide oversight, supervision, and support to field staff including CNAs, CHHAs, and other caregivers.
  • Conduct patient assessments and develop individualized care plans in accordance with professional nursing standards.
  • Collaborate with physicians and multidisciplinary team members to optimize patient care and outcomes.
  • Educate patients and families on health maintenance, disease prevention, and care expectations.
  • Ensure accurate documentation, compliance with regulatory requirements, and adherence to Joint Commission standards.
  • Monitor clinical performance and provide coaching, training, and mentorship to staff.
  • Act as a key liaison between patients, families, staff, and the BrightStar Care leadership team.
  • Support case management, scheduling, and follow-up to ensure continuity of care.

Requirements:

  • Active RN license in the state of Massachusetts.
  • Minimum 2 years of nursing experience (home health or acute care experience strongly preferred).
  • Prior leadership, supervisory, or case management experience is a plus.
  • Strong organizational, interpersonal, and communication skills.
  • Passion for delivering high-quality, patient-centered care.
