Job description

ABOUT THIS OPPORTUNITY:
Boundless Care Inc. is building a trusted roster of experienced caregivers in the Santa Clara/San Jose area to support private home care clients as requests come in. This is an on-call opportunity — ideal for caregivers who want flexibility while staying connected to meaningful care work. When a client need arises in your area, we'll reach out to matched caregivers on our roster. Open to both 1099 (independent contractor) and W2 (employee) arrangements.

TYPES OF CARE YOU MAY PROVIDE:
  • In-Home Care – Hourly, 24/7, or live-in support helping clients stay engaged and independent at home.
  • Alzheimer's & Dementia Care – Specialized memory care that preserves comfort, familiarity, and dignity.
  • End-of-Life Care/Hospice Care – Comfort-focused care and emotional support during advanced illness.
  • Respite Care – Flexible care that provides relief to family caregivers while ensuring consistent, reliable support.

ROLES & RESPONSIBILITIES:
  • Provide personal care assistance including bathing, dressing, grooming, and toileting.
  • Assist with mobility, transfers, and ambulation — including safe use of assistive devices.
  • Prepare nutritious meals and assist with feeding as needed.
  • Administer medications as directed and monitor for adverse reactions or changes in condition.
  • Monitor and document vital signs, noting any changes and reporting to the care team or supervising nurse.
  • Provide companionship and emotional support to promote client well-being and dignity.
  • Perform light housekeeping tasks to maintain a safe and comfortable home environment.
  • Implement individualized care plans and adapt to varying levels of acuity and client needs.
  • Communicate effectively with clients, families, and the Boundless Care coordination team.
  • Maintain accurate care logs, documentation, and timely incident reporting.
  • Uphold client confidentiality, dignity, and rights at all times.

WHAT WE LOOK FOR:
  • Valid California Home Care Aide (HCA) registration required.
  • Minimum 1 year of hands-on caregiving experience, preferably in home care settings.
  • Experience with high-acuity clients, including post-surgical and complex chronic conditions.
  • Experience working with clients with Alzheimer's or dementia (a plus).
  • Bilingual: English + any second language (a plus).
  • Reliable transportation and ability to travel within the Santa Clara/San Jose area.
  • CPR/First Aid certification (current).
  • Clear background check and satisfactory reference verification.
  • Strong communication skills and a compassionate, professional demeanor.
