Work Environment

As a Home Care Physical Therapist, you will have the flexibility to create your own schedule, working Monday through Friday, encouraging a work-life balance that meets your needs while ensuring our patients receive timely and effective therapy.

Key Responsibilities
  • Conduct thorough evaluations of patients in their home environment, assessing their physical and functional abilities to develop effective treatment plans tailored to their specific needs.
  • Utilize a variety of therapeutic techniques such as manual therapy, therapeutic exercises, and patient education to enhance mobility, reduce pain, and facilitate recovery.
  • Document each patient's treatment progress with precision, using electronic health records to track responses to therapy and adjust treatment goals as necessary.
  • Implement evidence-based practices and re-evaluate therapeutic strategies regularly to ensure optimal outcomes for patients recovering from injuries, surgeries, or chronic conditions.
  • Educate patients, families, and caregivers on therapeutic interventions and methods to promote recovery, including the use of assistive devices when necessary.
  • Maintain professional development through continuous education, networking with colleagues, and participating in relevant workshops to stay current with best practices in physical therapy.
Patient Population / Client Focus

The Home Care Physical Therapist will work with a diverse range of patients of all ages, including children and adults with various diagnoses. Our focus encompasses rehabilitation for individuals recovering from surgery, those dealing with chronic conditions, and reconditioning support for patients facing mobility challenges. This role demands sensitivity and adaptability to cater to the unique needs of each patient while fostering a supportive therapeutic relationship.

Qualifications
  • A Bachelor's degree in Physical Therapy or a related field is required.
  • Active state licensure in Physical Therapy is mandatory.

  • Proficient with computer systems and electronic documentation for accurate record-keeping.

Preferred Experience

Compensation and Benefits
