Clinical Responsibilities:

  • Provide comprehensive outpatient primary care to underserved populations including low-income, rural, homeless, and otherwise marginalized individuals.
  • Perform patient assessment, diagnosis, and treatment of acute and chronic conditions across the lifespan.
  • Order and interpret diagnostic tests, prescribe medications, and develop individualized treatment plans.
  • Delivery of care consistent with Aviva Health policies, procedures, and quality standards, emphasizing prevention and health promotion.
  • Document encounters in a timely and accurate manner in the electronic health record.
  • Respond to medical questions and provide patient education regarding medications, health conditions, and preventive care.
  • Collaborate with an interdisciplinary care team including medical assistants, nurses, behavioral health, pharmacy, and dental teams.
  • Participate in staff meetings, continuing education, and maintain required licensure and certifications.
  • Uphold Aviva Health's mission, vision, and values in all patient and staff interactions.

Qualifications:

  • Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in the state of Oregon.
  • DEA certification and BLS required.
  • One-year professional Advanced Practitioner experience in a clinic setting.
  • Demonstrated commitment to underserved populations and team-based care.
  • Strong interpersonal, communication, mentoring, and organizational skills.
  • Ability to work independently and collaboratively in a fast-paced environment.
  • Has knowledge and skills to provide services within scope of practice which may include the following age specific groups:
    • Neonates (birth to 1 year)
    • Children (1 year to 12 years)
