Job Overview: 

 
CareBuilders at Home is seeking a compassionate and experienced Registered Nurse (RN) to conduct in-home assessments for clients who require support services in the home. We proudly serve the Collin County area with a team of highly trained and dedicated staff. We would be honored to welcome you to the CareBuilders at Home family and work together to make a meaningful difference in the lives of those we serve. 

Duties 

  • Conduct Onboarding in home assessments of clients seeking support services at home 

  • Create non-medical care plans to include activities of daily living, light housekeeping, personal care, medication reminders, community transportation, respite care, etc. 

  • Onboard new clients and periodically reassess client's progress with the in-home care services 

  • Educate patients and their families on non-medical in-home care services and scheduling 

  • Utilize electronic medical record systems (AxisCare) to accurately document patient information 

  • Schedule initial caregiver visits 

Requirements: 

  • Pass drug screening 

  • Valid Driver's License 

  • Own Reliable Transportation to travel throughout Prosper and surrounding areas 

  • Proof of Car Insurance 

  • Social Security Card or Birth Certificate 

  • Negative TB skin test 

  • Provide three work related references
