Requirements:

  • Active Registered Nurse License in the state of Georgia
  • Tuberculosis (TB) screening/QuantiFERON/Chest X-Ray (within 1 year)
  • Proof of U.S. work authorization (valid, unexpired documents: government issued ID)
  • Reliable transportation

Responsibilities:

  • Conducting Start of Care (SOC) visits to evaluate medical, physical, and emotional needs and creating individualized care plans.

  • Supervising and supporting Personal Care Aides (PCAs) and other care team members.
