NOW HIRING: Bilingual Case Manager (English/Spanish)

We are seeking two compassionate and motivated Case Managers to provide Rapid Re-Housing services to veterans, young adults, and single adults experiencing or at risk of homelessness. This role is ideal for someone passionate about helping individuals achieve long-term housing stability through advocacy, case management, and community resources.
Responsibilities
- Manage a caseload of approximately 18-20 participants.
- Conduct assessments, intakes, and develop individualized housing stability plans.
- Provide office, community, and home-based case management services.
- Utilize Critical Time Intervention (CTI), Motivational Interviewing, and Harm Reduction approaches.
- Assist clients with housing searches, apartment viewings, landlord advocacy, and housing placement.
- Conduct housing inspections and help resolve landlord-tenant disputes.
- Connect participants to employment, education, healthcare, benefits, and community resources.
- Complete accurate case notes and maintain electronic client records.
- Provide crisis intervention, follow-up services, and ongoing support to promote housing stability.
- Participate in case conferences, trainings, and community outreach initiatives.
Qualifications
- Bachelor's Degree in Human Services or a related field.
- Bilingual in English and Spanish is REQUIRED.
- Experience working with individuals experiencing homelessness, mental health challenges, substance use, or trauma is preferred.
- Experience working with veterans, young adults, or LGBTQ+ populations is a plus.
- Knowledge of NYC housing and community resources preferred.
- Strong organizational, communication, writing, and computer skills.
- Ability to work independently in a fast-paced environment and conduct field visits throughout NYC.
