BHH (Behavioral Health Home)

Pay: $24.00โ€“$28.00 per hour
Work Location: Community-based; local travel required

Join Our Team
We are seeking a compassionate and motivated Mental Health Practitioner to provide Servicesย to adults living with mental health conditions.

In theseย roles, you will work directly with clients in their homes and community settings, helping them develop practical skills that support independence, recovery, and stability. You will work as part of a supportive team focused on improving the lives of individuals in the community. An agency vehicle is provided for work-related travel.

Key Responsibilities
Provideย rehabilitation,ย skills training and med management services in community settings
Support clients in developing skills for independent living, symptom management, communication, and daily functioning
Implement individualized treatment plans in collaboration with the clinical team
Meet with clients in homes or community locations
Travel locally to meet with clients (agency vehicle provided)

Qualifications
Bachelorโ€™s degree in Social Work, Psychology, or a related behavioral health field
OR
2,000 hours of supervised experience in the delivery of mental health services to individuals with mental illness
