Location: Honolulu, HI (Must have reliable transportation)

Weโ€™re a growing care organization in Honolulu dedicated to home health, fall prevention, and empowering residents in Assisted Living Facilities (ALFs). Weโ€™re currently hiring a licensed Physical Therapist (PT) or Occupational Therapist (OT) whoโ€™s passionate about improving lives through quality, in-home and community-based therapy.

What Youโ€™ll Do:

  • Deliver physical or occupational therapy services to clients in their homes.

  • Conduct initial and follow-up assessments, including fall risk evaluations.

  • Perform community outreach via ALF presentations and expos.

  • Oversee client progress and ensure top-notch care.

  • Represent the organization in community events and promote wellness initiatives.
