Weekend LPN needed in Valatie, NY. Work 4 shifts, get paid for 5 with premium Baylor rate, providing quality resident care.

Job Details
  • Location: Valatie, NY (25 minutes from Albany)

  • Position Type: Full-Time (Weekend Baylor Program)

  • Shifts: Saturday & Sunday, 15-hour shifts

  • Pay Range: $33.75–$44.50/hr (with Baylor rate applied)

  • Requirement: Must complete all shifts to earn Baylor rate

About the Facility

The Grand Rehabilitation and Nursing at Barnwell is part of The Grand Healthcare System, a network of advanced rehabilitation and long-term care facilities. Our focus is on high-quality, resident-centered care and professional growth.

Position Summary

The Weekend Baylor LPN provides direct nursing care on weekends, manages treatments and medications, supervises CNAs, and documents resident care. This role offers premium pay for completing all assigned weekend shifts.

Primary Responsibilities

Patient Care & Support

  • Complete the nursing process to promote resident health

  • Provide physical, educational, and emotional support to residents and families

  • Supervise assigned CNAs to ensure high-quality care

Documentation

  • Maintain accurate resident care records in the electronic medical system

  • Ensure CNA documentation is complete and accurate

Medication Administration & Reporting

  • Administer medications according to protocol

  • Provide and receive written and verbal shift reports

Team Collaboration

  • Work closely with CNAs and the interdisciplinary team to coordinate care

  • Support quality, timely, and resident-centered care

Qualifications
  • Current NY LPN license

  • CPR certification

  • Experience in a nursing home or hospital preferred

  • Strong communication and clinical skills

  • Ability to work independently and as part of a team
