Job Title: Clinical Pharmacist - Inpatient Pharmacy
Location: Glenwood Springs, CO
Job Type: Full-Time
Work Model: Onsite
The Clinical Pharmacist - Inpatient Pharmacy is responsible for preparing, dispensing, monitoring, and optimizing medication therapies for hospitalized patients within a rural acute care hospital environment. This role supports safe, effective, and evidence-based medication management while collaborating closely with physicians, nursing staff, and interdisciplinary healthcare teams.
The ideal candidate will possess strong inpatient clinical pharmacy experience, hospital pharmacy operations expertise, medication therapy management capabilities, and experience supporting acute care and emergency response environments.
Key Responsibilities
Clinical Pharmacy Services
  • Interpret and verify medication orders accurately
  • Maintain computerized patient medication profiles
  • Monitor medication therapy for:
    • Contraindications
    • Drug interactions
    • Allergies
    • Appropriate dosing
  • Perform pharmacokinetic dosing per hospital protocol
  • Review and interpret culture & sensitivity (C&S) data for antibiotic appropriateness
  • Recommend therapeutic alternatives and medication optimization strategies
  • Provide clinical consultation and drug information to providers, patients, and families
  • Document clinical interventions and pharmacy activities accurately
Medication Preparation & Distribution
  • Compound and dispense medications including:
    • IV admixtures
    • Chemotherapy preparations
    • Parenteral nutrition products
  • Manage medication inventory and controlled substances in compliance with regulations
  • Support formulary management and therapeutic substitution initiatives
  • Maintain safe medication-use processes across inpatient care areas
Quality & Safety
  • Detect and report adverse drug reactions
  • Participate in medication safety and quality improvement initiatives
  • Conduct audits, quality monitoring, and documentation reviews
  • Ensure compliance with:
    • Federal regulations
    • State pharmacy laws
    • Hospital policies and protocols
Education & Collaboration
  • Participate in orientation, education, and training programs for:
    • Pharmacy staff
    • Nursing staff
    • Medical staff
  • Collaborate with physicians, nurses, and interdisciplinary healthcare teams
  • Participate as a member of the Code Response Team
Must-Have Qualifications
  • Graduate of an ACPE-accredited School of Pharmacy
  • BS Pharmacy or PharmD Degree
  • Current Colorado Pharmacist License OR eligibility to obtain
  • BLS Certification (AHA)
  • ACLS Certification (AHA)
  • Health system / hospital pharmacy experience required
  • Strong inpatient clinical pharmacy experience
  • Experience with:
    • Medication reconciliation
    • Drug therapy management
    • Clinical intervention documentation
    • Code Blue / emergency response support
  • Strong communication and interdisciplinary collaboration skills
Preferred Qualifications
  • 3-5 years of hospital pharmacy management or leadership experience preferred
  • Experience in rural hospital settings preferred
  • Experience supporting acute care and emergency response environments
  • Strong antimicrobial stewardship and pharmacokinetics experience
